During your appointment, we will:
Complete a FRAIL Scale assessment (quick, easy screening tool)
Conduct strength and mobility tests (like handgrip and sit-to-stand)
Review your medications and vaccinations
Provide personalised advice on nutrition, exercise, and home safety
Discuss mental health and wellbeing
Identify services and supports available to you
We use specialised tools (PENCAT, CSIRO model) to identify patients at higher risk of hospitalisation and prioritise early care.
We offer education, home safety advice, personalised exercise handouts, and medication reviews to help you thrive at home.
We work with local gyms, physiotherapists, and council programs to connect you to strengthening classes and community activities.
